Compu-Tools Adult Alcohol and Drug Testing Questionnaires:

Note:  Most Clinicians use the Compu-Tools Questionnaires and their Assessment Process is as follows:

·         The Client completes the Demographic information, non-disclosure agreement, Drug Self Tests and the Psychological Tests on the Compu-Tools questionnaire. (20-30 minutes)

·         The Clinician reviews the information completed by the Client.

o        If the Alcohol/Drug Self Tests shows that the Client admits to an Alcohol or Drug problem, the Compu-Tool Assessment Questionnaire can be completed by the Client or by the Clinician and Client. (15-30 minutes)

o        If the Alcohol/Drug Self Tests show no evidence of a problem, the Compu-Tools Assessment Questionnaire should be completed by the Clinician (20-30 minutes)

o        After Completing all questions, the Clinician enters the Client question responses into the computer software (8-12 minutes)

o        The Assessment Software scores all Compu-Tools test scales and prints a completed Drug Assessment (2-4 minutes)

The Compu-Tools Assessment Questionnaire is a critical component of the drug assessment process.  The most effective Clinicians use the following techniques with their clients:

·         Review the reason(s) the Client was motivated to participate in the Alcohol and Drug Assessment Process.

·         The demographic information that has been completed on the questionnaire

·         The results of the two self tests included on the Compu-tools Drug Assessment questionnaire that can be completed quickly by the Client and are easy to manually score. (A quick count of positive responses)  These tests will provide an indication of the Client’s perspective on his, or her, alcohol or drug use.  If the Client admits to having a problem with drugs, denial will be a minor issue when the Compu-Tools Drug Assessment section of the questionnaire is completed.

·         Each Client response to questions asked on the Compu-Tools Drug Assessment Questionnaire are carefully noted by the Clinician and supporting notes made on the Questionnaire itself.  When a client response seems to contradict an earlier response, the Clinician will refer back to notes made and assist the Client in providing an honest response.  (E.g. “you indicated that no family or friends have been affected by your behaviors - although you have been incarcerated for a drunk while driving violation.”  Were you bailed out? By Whom? Etc…)

·         The next to last page of the Compu-Tools questionnaire contains a description of all scoring information. If it becomes necessary to describe how the computer is generating problem categories and protects against “false positives” this page will provide the information.

·         At the conclusion of the assessment, the chart on the last page of the Alcohol and Drug questionnaire will allow the Clinician and the Client to manually check each test result with the norms shown on the chart.

·         The completed questionnaire and the Clinician notes become part of the Client’s permanent record.  It can provide meaningful information if the Client returns for reassessment.

A review of the printed questionnaire will show why manual scoring of the variety of tests is non practical and why the software must be used in conjunction with the written document.
