Performance Contract:
The Contract includes activities that the participant has
agreed to complete. These conditions will be compared with the activities
reports that are provided to the Monitoring Service.
Activity Forms:
These forms contain information about the participants activities that
were agreed upon in the Performance Contract. The information collected will be available for retrieval
on the day following receipt by our monitoring service and include:
- Mailed to our post office box
- Faxed to our toll free fax number
- Sent to our e-mail address
- Entered directly onto our web-site
Activity Reports:
Monthly or bi-monthly reports of client activities can be automatically sent by mail or email. Timely
reports by date range can also be obtained from the imr.com web site. This report can be downloaded
or printed after being viewed. If actual copies of faxed or mailed activity forms are desired, these can
also be downloaded from our web-site.
Alerts:
Special email alerts based upon activity reporting can be subscribed
to if appropriate arrangements have been made.
Statistics:
Individual statistics are available by referral source from our activity database. Outcome
statistics can also be developed for those customers who are willing to provide historical information on incoming participants. |
